
The Johnson Center offers several levels of care. An individual can be admitted from home, another facility, or from a hospital or rehabilitation center.
We recognize that the admissions process can be confusing and time consuming. We invite you to start the process as early as possible. Contact the

for any questions of concerns.
Each level of care must meet specific guidelines for admission. The following is a list of items to be completed for admissions to our skilled or long term care beds (this includes permanent, temporary, or respite care residents).

Choose a Johnson Center participating physician (a list can be provided for you). |

Copy of Social Security Card, Medicare Card, Military ID, and secondary insurance cards |

Advanced Directives, Living Wills, Durable Power of Attorney (POA), and Do Not Resuscitate (DNR) forms. If you do not have these prepared, you may make an appointment with the Social Services Coordinator for more information and help with these forms |

Terrace Residents need a psychiatric consultation within 60 days prior to admission |
There are numerous items that we will also need from the referring hospital or primary care physician in addition to the items requested from the resident or his/her family. These include History and Physical Examination and Chest X-Ray within 30 days of admissions.

Transitions between the levels of care, such as from assisted living to independent living or assisted living to nursing care or skilled nursing will be based on the resident’s medical needs. This is based on thorough assessments made by our professional team, physicians involved and any new medical developments that resulted in hospitalization or change in health status of the resident. The resident and family members involved in the care of the resident will be at all times notified of any changes that are recommended for the best quality of care for the resident.
