We encourage you to make each day your own, by providing you with the right amount of care to meet your needs in a warm and friendly environment.The Johnson Center is an 89 bed facility that offers Assisted Living, Nursing Care, Respite Care, and Skilled Nursing Care in a compassionate and caring setting. In each level of care, residents enjoy a private room and bath along with a wide variety of other amenities. West Falls Center offers Assisted Living services with 40 apartments and 4 respite units.

Sometimes an individuals needs for assistance extend beyond what a retirement community, assisted living facility, or loving family member can provide. We created our Nursing area with that in mind. Nursing or custodial care can be provided on either a long-or short-term basis. Our well trained nurses are on duty 24 hours a day to provide you with the gentle, supportive and individualized care you need, including:
- Restorative nursing after an illness, injury, or surgery, or stroke
- Palliative care
- Pain management
- IV therapy
- Wound care
Skilled care, as defined by Medicare is health care given when you need skilled nursing or rehabilitation staff to manage, observe, and evaluate your care. People usually do not remain in a Skilled Nursing Facility until they are completely recovered, Medicare covers certain skilled services that are needed daily on a short-term basis or up to 100 days. Medicare will cover skilled care: if an individual has Medicare Part A, has had an inpatient hospital stay for three consecutive days (not including the day you leave the hospital), enters skilled within 30 days of discharge from the hospital, and if the individual’s physician has decided that he/she requires daily skilled care. Medicare covers the following services when an individual qualifies for skilled care: (Medicare covers therapy services if they are needed to meet an individual’s health goal.)

At The Johnson Center has 13 beds certified by Medicare for skilled nursing services.


The Terrace is a well-secured, private pay unit with twelve beds for residents who have been diagnosed with Dementia or Alzheimer’s disease. The Terrace has a dining area, a living room area and a secured outdoor patio with gazebo.

Respite Care refers to short-term stays on either our Nursing Unit or Assisted Living Unit. Respite care provides caregivers additional help when needed by giving them temporary relief of care taking responsibilities. It also gives the resident a break from being dependent upon their families for care. At these times,
