Covered Services - Skilled Nursing

Our facility's Medicare payment rate includes these following services, and no additional charges will be made to the resident.

Non-Covered Services - Skilled Nursing

The following services are not included in our Medicare payment rate.  Charges for these services will be reflected on your monthly statement (Ask the Admissions Coordinator for a list of billable items). Charges for these services will be billed directly to the resident, or to the individual that has control of or access to the resident's funds by the service providing or performing the requested service.

Personal Physician     

Nursing floor residents:  State and Federal guidelines require you to have a personal attending physician who will oversee your care in the facility. If your physician will not see you in the facility and you are unable to find one that will, the staff can provide a list of physicians that offer service in The Johnson Center.

Assisted Living residents: You may retain a physician outside of the facility, provided the physician is available by phone to give orders.

• Dining Services

Alcoholic Beverages

Consumption of alcoholic beverages is allowed with a physician's order.  Alcoholic beverages will be stored only in the medication room and must be provided by family unless it is served at an activity social function.

Diet

Well balanced meals are planned by our Dining Services Director, Executive Chef, and Sous Chef in accordance with your needs as determined by your physician's orders.  A Certified Dietary Manager will assist with food selection to ensure nutritional adequacy and compliance with prescribed diets.  The Certified Dietary Manager will interview you and your family upon admission to determine your previous eating habits.  All meals are menu-select.  If you do not like certain foods, substitutes are available.  Reduced portions can be provided upon request in accordance with dietician intervention and a doctor’s order. The Johnson Center contracts with a Registered Dietician who reviews the dining program and provides oversight to the Nursing unit.

Dining Room

To promote social interaction, all meals are served in the dining room.  Should your medical condition preclude taking meals in the dining room, you may temporarily request meal service to your room.

• Environmental Services

Housekeeping

Housekeeping services are provided by the facility and included in your daily rate. For specific questions or concerns regarding housekeeping services, please contact the Environmental Services Director at (703) 404-5122.

Laundry

Laundry service is provided at no additional charge. All resident clothing is to be labeled by the family with a permanent marker.  We ask that a flame-retardant, airtight container be provided to store soiled clothing.  Laundry services are provided by the facility, however, your responsible party may also choose to do your laundry.    For your convenience, laundry equipment is available for use by your responsible party.  You or your responsible party is to provide laundry detergent.

For any maintenance requests, you may notify a staff member or you may contact the work order line directly at (703) 404-5126.

• Rehabilitation Services

An important part of The Johnson Center’s full spectrum of health services is our rehabilitation program. We offer a variety of therapies, in individual or group settings, to encourage independence and support recovery. Our rehabilitation team completes a careful evaluation of each resident, to determine what befits may be derived through appropriate therapies.

Advance Directives

Advance directives are written documents made in advance of a serious illness that state your choices for health care or name someone to make those choices for you if you become unable to make these decisions.  Through advance directives such as a living will, durable power of attorney for health care and/or health care surrogate, you can make legally valid decisions about your future medical treatment.  These written instructions are recognized under State law.

Bed Holds

In the event that a permanent resident of The Johnson Center is transferred per a physician’s order from the facility to a hospital or other health care facility, The Johnson Center will reserve a bed for that resident at the cost of the daily rate per day minus the cost of meals, until notified by the Responsible party to discontinue the bed hold.

Care Conferences   

An individualized physician approved care plan is developed for you upon admission and is revised to reflect any changes in physician orders or your care needs.

Spiritual Needs

Your church representative is always welcome and encouraged to visit you.  Regular religious and spiritual activity programs are arranged by the Chaplain and are listed on a monthly spiritual activities calendar as well as the general activity calendar.

Grievance Procedure

The Johnson Center at Falcons Landing strives to deliver services to all residents with efficiency, in a professional manner, and with compassion.  However, we realize that concerns will sometimes arise.

Discharge Against Medical Advice

Our facility cannot hold a resident against his/her will.  Should a resident wish to be discharged, every effort will be made to contact the legal guardian or representative (sponsor) before the resident is discharged.  Documentation of such discharges is recorded in the resident’s medical record.

Ombudsman

"Ombudsman" is a Swedish term for a person who acts as a citizen representative.

Private Duty Aides

Private Duty assistance is accepted at The Johnson Center pending compliance with our facility’s policies and procedures.  If you are interested in hiring private assistance, please contact the Social Services Coordinator at (703) 404-5214 for approval.
